Obtain an application form BMV 4826 by downloading it from the BMV Web site, from any local Deputy Registrar license agency, by calling 614-752-7800 or 1-800-589-TAGS (8247).
Take the completed application along with the required processing fee to:

Second placards (if issued) are an additional $3.50.

A second placard may be issued if applied for separately and states the reason why a second placard is needed. Second issue placards expire on the same date as the original placard, regardless of issuance date
New Placard
Temporary placards are red in color and expire on the date prescribed by the health care provider. Temporary placards CANNOT be renewed.
Permanent placards are blue in color and will expire on the date prescribed by the health care provider. A renewal application will be mailed to you 60 days prior to the expiration date.

Replacements Placard
Your replacement placard will expire on the same date as the original placard, regardless of the issuance date
New Placard
Disability placards are issued to organizations, public or private, which transport persons with disabilities on a regular basis in a motor vehicle that has not been altered with special equipment for use by persons with disabilities.
Renewals Placard
Placards must be renewed every five years. A renewal application will be mailed to you 60 days prior to the expiration date.
Replacements Placard
Your replacement placard will expire on the same date as the original placard, regardless of the issuance date.
